What you need to submit

- 2012 Application Form
- Enrollment fee of $25.00 (non-refundable) and $100 deposit (refundable if made prior to 2 weeks before the start date of Boot Camp.) Each class must have a minimum of 10 enrolled students.
- Each piece of paper should have your name written on it in the event something gets separated from your file.
- A minister's (or other spiritual leadership covering) Letter of Recommendation.
